Fill out this application when applying for new phone service or to restart services after your account has been disconnected for 90 days. If your telephone service is going to be installed in a newly built home, you will also need to fill out a Service Line Agreement.

Service Line Agreement Application (SLA)
GRTI requires that you fill out the Service Line Agreement (SLA) when applying for new telephone service in a home on tribal or allotted lands. It is needed for all newly constructed homes in Gila River. It requires the signature of both the owner and lessee.
Enhanced Lifeline & Link-up Application
Enhanced Lifeline and Link-up is a federal program available to all households on Tribal lands. When you qualify for Link-up, it pay $45 of the GRTI installation fee, bringing it to $30. When you qualify for Link-up you are automatically qualified for Enhanced Lifeline. Enhanced Lifeline gives you a monthly discount of $18.50 every month. Fill out and return to GRTI.

Start New Service
To start new service please submit the following to GRTI Customer Service:
- GRTI Telephone Application (Click here for application)
- Service Line Agreement - required if living on tribal or allotted land (Click here for SLA)
- Picture ID
Installation Fees
There is a one-time installation fee of $75 dollars. There are several options of payment.
- Pay at the time you submit application
- Billed to your account on first bill or three payments of $25 for three months.
Monthly Recurring Fees
Your telephone will always have the following charges on your phone bill regardless of your telephone usage. These charges are implemented by the Federal Communications Commission (FCC) that GRTI is governed by.
